Botox for bruxism in Wrocław: qualification and treatment of chewing muscle tension
Bruxism means repetitive activity of the chewing muscles: clenching, grinding or bracing the jaw while awake or asleep. Botulinum toxin may, in selected adults, limit excessive muscle activity and the complaints associated with it. It does not, however, remove all the causes of bruxism, it does not repair damaged teeth and it does not replace dental diagnosis, the assessment of temporomandibular disorders and of sleep disorders.
Teeth clenching, jaw pain and a morning headache do not automatically mean a single diagnosis
Awake bruxism and sleep bruxism are separate behaviours of the chewing muscles. They may coexist, but they require different ways of observation. Facial pain, tooth wear, clicking in the temporomandibular joint and headaches also have other possible causes. For that reason the decision to inject should not rest solely on a feeling of “strong masseters” or on a photograph of the lower face.
Awake bruxism
It often takes the form of clenching, bracing or keeping the teeth in contact during concentration and stress. It helps to recognise the triggering situations and to learn to relax the jaw consciously.
Sleep bruxism
The patient may not remember the episodes. Information may come from a partner’s account, morning tenderness or a dental assessment. In selected cases a consultation in sleep medicine or a sleep study is needed.
Damage to the teeth
Wear, cracks, sensitivity or damage to restorations require a dental examination. The toxin does not rebuild enamel and does not replace protection of the teeth.
Temporomandibular disorders
Joint or muscle pain, restricted mouth opening, clicking and a change in the bite require differentiation. Muscular and joint symptoms may occur together, but they are not the same.
Headaches and facial pain
Morning pain in the temple or a feeling of tension may accompany overload of the chewing muscles, but it is not specific to bruxism. A new, sudden or unusual headache requires a separate medical assessment.
Sleep and breathing
Loud snoring, observed pauses in breathing, awakenings and daytime sleepiness may indicate sleep-related breathing disorders. An injection into the masseters neither diagnoses nor treats sleep apnoea.
The order matters: the doctor assesses the muscles and the indications for pharmacological treatment, the dentist protects the teeth and assesses the bite, and a physiotherapist or a sleep medicine specialist may be needed depending on the dominant symptoms. A well-run plan does not pretend that one procedure solves every one of these problems.
What does qualification for treatment with botulinum toxin look like?
- History. The doctor asks about when the symptoms occur, pain, restricted jaw movement, the condition of the teeth, earlier treatment, neurological and neuromuscular diseases, medicines, pregnancy, breastfeeding and problems with swallowing or breathing.
- Examination of function. The areas of the chewing muscles, tension and tenderness, symmetry, jaw movement and the character of the complaints are assessed. A wide lower face alone does not confirm bruxism.
- Setting the aim. The aim may be to limit painful muscular overactivity, not to stop every episode of grinding or to promise a change in facial features.
- Assessment of other methods. The doctor establishes whether a dental consultation, treatment of sleep disorders, physiotherapy, behavioural modification or another kind of treatment is needed first.
- An informed decision. The patient receives information about the expected scope of action, the limitations, the possible adverse effects, the character of the use of the product and the plan of follow-up.
What can botulinum toxin achieve in overactivity of the chewing muscles?
Botulinum toxin type A inhibits the release of acetylcholine at the nerve endings and temporarily reduces the activity of selected muscles. Studies indicate that it may reduce the force of contraction, the electromyographic activity and some of the pain. At the same time the results of the studies are varied, the groups of patients small and the protocols non-uniform. Treatment therefore requires an individual assessment of benefit and risk.
It may limit the overload of the muscles
In a properly qualified patient, weakening excessive muscle activity may reduce the tension, tenderness or pain associated with clenching.
It does not remove all episodes
A controlled sleep study showed a reduction in the intensity of the contractions, but not in the occurrence of the bruxism episodes themselves. That is an important distinction when setting a realistic aim.
It does not protect the teeth mechanically
A reduction in muscle force does not rebuild enamel or restorations. When there is a risk of damage, a protective splint chosen by a dentist may be needed.
It is not the only treatment
Bruxism may require dental, behavioural or physiotherapeutic management or treatment of sleep disorders in parallel.
The effect is temporary and individual
The response depends on the diagnosis, the muscle activity, the anatomy, the product and the earlier treatment. A particular strength or duration of action cannot be guaranteed.
Monitoring of function is needed
After treatment not only the pain is assessed, but also chewing, facial expression, symmetry and adverse effects. A weakening of the bite force is part of the pharmacological action, and not always a desirable aim.
Status of the indication: the use of botulinum toxin in the chewing muscles to limit the symptoms of bruxism may be a use outside the registered indications of a particular medicinal product. The doctor assesses the clinical justification and the current Summary of Product Characteristics and gives the patient the information needed for an informed decision.
What other kinds of treatment are considered?
A dental splint
An individual splint may limit mechanical damage to the teeth. It does not always stop muscle activity, so its fitting and follow-up are handled by a dentist.
Changing daytime habits
Reminders, breaks at work and learning the resting position of the jaw help to limit unconscious clenching during concentration. The teeth should not remain constantly in contact.
Physiotherapy
With restricted movement, muscle pain or coexisting temporomandibular disorders the doctor may recommend physiotherapy directed at the function of the masticatory system.
Dental treatment
Cracks, sensitivity, cavities and damaged restorations require separate management. A permanent change of the bite is not made solely on the basis of a suspicion of bruxism.
Assessment of sleep
With snoring, apnoeas or marked sleepiness, diagnosis of sleep disorders may be needed. Treating such a problem has a different aim from weakening the chewing muscles.
Stress and aggravating factors
Techniques for reducing tension, regular sleep and limiting late caffeine, alcohol and nicotine may support the plan. This does not mean that bruxism is “just stress”.
What has to be discussed before the injection and after the procedure?
Tell the doctor about earlier procedures with botulinum toxin, the name of the medicine, the date of administration and any antibiotics, anticoagulants and medicines affecting neuromuscular transmission that you take. Also report neurological diseases, swallowing disorders, respiratory diseases, pregnancy and breastfeeding. Different products containing the toxin should not be treated as equivalent.
When is the procedure not performed?
A contraindication may be, among others, hypersensitivity to the components, an infection at the site of the planned injection or certain neuromuscular diseases. The full list refers to the product chosen.
Possible local effects
Pain, tenderness, swelling, redness, a haematoma or transient weakness of the muscle may occur. In the masseter area chewing and the symmetry of the smile also matter.
Symptoms requiring urgent help
Disturbances of breathing, swallowing or speech, increasing generalised weakness or a severe allergic reaction require immediate medical help. In a life-threatening situation call 112.
After the procedure
The patient receives individual recommendations. Do not massage the area on your own, undertake further injections or assess the need for a correction on the basis of instructions from the internet.
Follow-up
The follow-up date depends on the clinical condition. The doctor compares the symptoms and function with the starting point and decides whether observation, further diagnosis or another kind of treatment is more appropriate.
Pregnancy and breastfeeding
The product characteristics may advise against use in pregnancy and breastfeeding because of limited data. Pregnancy, its possibility and breastfeeding must be reported before qualification.
When can repeat administration of the toxin be considered?
If the first procedure brought a clinically significant reduction of the complaints and there were no adverse effects excluding further treatment, the doctor may consider repeat administration of the toxin. At subsequent visits the doctor compares the severity of the symptoms, the function of the masticatory system and the tolerance of treatment. The date of a further procedure should not follow solely from the passage of a set time.
Repetition after renewed qualification
Every further procedure is preceded by an assessment of the current complaints, chewing function, the earlier effect, adverse effects and changes in health.
Not according to a rigid date
The return of tension should not be predicted to the week. The interval must take into account the action of the previous therapy and the rules proper to the product used.
The possibility of correcting the plan
If the benefit is small, weakness of chewing appears or the diagnosis changes, the doctor may refrain from repetition and recommend another kind of treatment.
Repeatability does not mean necessity. The doctor chooses the scope of treatment so as to reduce the complaints without excessive weakening of the chewing muscles. Renewed qualification protects against unnecessary injections and makes it possible to take into account dental treatment, physiotherapy and a change in aggravating factors.
Botox for bruxism Wrocław: FAQ
Does botox treat the cause of bruxism?
Not always. It may reduce the activity and force of selected muscles, but it does not automatically remove mechanisms connected with sleep, stress, behaviour or temporomandibular disorders.
Will the procedure stop teeth grinding?
That cannot be guaranteed. Studies point above all to a reduction in the intensity of muscle contractions; episodes of sleep bruxism may still occur.
Do I need a splint before the consultation?
No. Bring your splint if you already use one. The need to make or adjust a splint is assessed by a dentist after examining the teeth, the periodontium, the bite and the joints.
Do wide masseters mean bruxism?
No. The structure of the face and hypertrophy of the muscles may have different causes. The diagnosis of bruxism rests on the symptoms, the history and the examination, not on appearance alone.
Is pain in the temporomandibular joint an indication for botox?
Not automatically. The pain may come from the muscles, the joint or other structures. The dominant diagnosis must first be established and conservative methods assessed.
Will the procedure change the contour of the face?
A reduction in the activity and volume of the masseter muscles may affect the contour of the lower face, but the degree of this change differs between patients and cannot be guaranteed. In treating the complaints the priority remains the correct function of the masticatory system.
How long does the effect last?
The action is temporary, but its strength and duration differ between patients and products. The need for follow-up or repetition is decided by clinical assessment, not by a fixed date.
Can the procedure be repeated regularly?
It may be considered after renewed qualification, if earlier treatment brought benefit and was well tolerated. Further administrations are not made automatically or more often than the rules of the particular product allow.
Is sleep diagnosis needed?
Not for everyone. It is particularly important with suspected apnoea, marked sleepiness, unusual movements during sleep or an unclear diagnosis. The decision is taken by the appropriate specialist.
What should be brought to the visit?
A list of medicines, diseases and earlier procedures, the name and date of the last administration of toxin, dental documentation, the splint in use and a short note of the situations in which the clenching is strongest.
Is the price of the procedure on this page?
Current fees are published in one HappyDERM price list. The final scope of treatment depends on qualification and should not be set solely on the basis of price.
